All of the web sites listed below provide free access to MEDLINE, the world's largest database of medical abstracts, including thousands of journal articles dealing with diabetes.


MEDLINE is a service of the U.S. Government's National Library of Medicine (NLM) in Bethesda, Maryland. The NLM is the world's largest research library in a single scientific and professional field. Its computer-based Medical Literature Analysis and Retrieval System (MEDLARS) was established in the early 1960s for the production of bibliographic publications and for conducting individualized searches of the literature for health professionals. Subsequently, MEDLARS has come to represent a family of databases of which the MEDLINE (MEDlars onLINE) database is the most well known. Essentially Index Medicus online, MEDLINE enables individuals and organizations with computer terminals to query the NLM computer's store of journal article references on specific topics.
MEDLINE is the NLM's premier bibliographic database covering the fields of medicine, nursing, dentistry, veterinary medicine, the health care system, and the preclinical sciences. It contains bibliographic citations and author abstracts from more than 3,900 current biomedical journals published in the United States and 70 foreign countries and has more than 9 million records dating back to 1966. Coverage is worldwide, but most records are from English-language sources or have English abstracts.


MedFetch

A new and different way to access the 9 million citations in MEDLINE and Pre-MEDLINE is MedFetch. With its Automated MEDLINE Query (AMQ) you can create and run your own query repeatedly. For example, you can search for all new citations about diabetes, and abstracts of these articles will arrive by e-mail every week or every month at your choice. Each is databased so next time the search is run only new citations are mailed out. MedFetch recently made a major software upgrade that allows searches to be delivered in real-time to your e-mail address immediately after they are created. The follow-up searches are then run weekly or monthly, according to the frequency you select. The upgrade also provides translation services in real-time for up to 5K of data. You can receive the citations requested in English, French, Spanish, German, Italian, or Portuguese. MEDLINE Interfaces and Related Resources" is the on-line version of an appendix from MEDLINE: A Guide to Effective Searching , a recently published book by Brian S. Katcher (Ashbury Press, San Francisco, 1999).
"One thing I didn't mention specifically in the book is the depth of indexing for diabetes-related papers, There are specific Medical Subject Headings (MeSH) for Diabetic Nephropathies; Diabetic Neuropathies; Diabetic Foot; Diabetic Retinopathy; Pregnancy in Diabetes; Diabetes, Gestational; and others-in addition to the obvious Diabetes Mellitus, Non-Insulin-Dependent; and Diabetes Mellitus, Insulin-Dependent. They're all in the on-line MeSH."

Medical Matrix

Medical Matrix: A ranking 15 different free and fee MEDLINE services available on the Web, including access restrictions, dates covered, fees for document delivery, navigation tools, and utilities for expanding searches to medical concepts. Of these sites, eight are free and seven are fee-based.


PubMed

PubMed, a top-rated MEDLINE service, is the National Library of Medicine's experimental search service. PubMed is free and provides immediate access to all abstracts published since 1966.

U.S. Library of Medicine

In addition, The U.S. Library of Medicine offers its on-line NLM Technical Bulletin, a bimonthly newsletter for MEDLARS online searchers. Through the Technical Bulletin, NLM keeps command language searchers apprised of changes and enhancements to its systems. Among other things, you can learn about new databases, search features, and changes to MeSH vocabulary.

 

Medscape
Medscape offers free MEDLINE access for all articles published since 1966. The service is fast and has a powerful search engine allowing a variety of types of searches. It returns results in a choice of four formats. A search for a particular term or phrase will also let you search for that term or phrase automatically in Medscape's free full-text articles or the Medscape Bookstore. You can order full-text documents by mail, fax or express for a basic fee of $8 plus special delivery charges and copyright charges.


HealthGate

HealthGate also has free access to MEDLINE and requires no registration. But it provides only those articles published since 1990.


Healthworks Online

MEDLINE abstracts published since 1966 are also available free to all UK/USA residents on MEDLINEplus from Healthworks Online. Full-text articles cost $8.50 plus copyright royalty fees.

Web-Based Diabetes Software


AIDA On-line offers the opportunity to simulate the effects of changes in insulin and diet on the blood glucose profile of a "virtual diabetic patient." You can run simulations without charge in your Web browser window. The simulator allows you to see the effect of changes in insulin and diet on the blood glucose profile of example patients with diabetes. AIDA On-line provides 40 case scenarios with different problems to be solved by interactively simulating the "virtual diabetic patient." Users can add more cases. The approach is not intended for individual patient glycemic prediction or therapy planning, but is offered as an interactive educational/self-learning tool. A downloadable version of the simulator is also available and is linked above in the "Shareware and Freeware Windows Software" and "MS-DOS Software" sections.


Bayer Care: Blood Sugar Log
The Online Blood Sugar Log lets you keep track of all your blood sugar test results on the Web.


CBSHealthWatch
CBSHealthWatch by Medscape has a bare bones and hard-to-find blood glucose tracker on its Web site. To get to it you need first to register then click on "Daily Diary" and then on "Blood Glucose."


DiabetEASE
DiabetEASE Inc. in Ottawa, Ontario, Canada, is the first site to provide free on-line software that allows you to automatically upload data from any one of several meters to track your blood glucose levels. Just connect your meter to one of your computer's serial port with the meter's cable, click a couple of times, and your data is displayed in easy-to-read graphs that can be customized to show your blood glucose levels according to day, week, month, or by events such as exercise or meals. The program also graphs insulin. Use the TheraSense FreeStyle.


Diabetes HomeCare Center
Rose Technologies Inc. in Bellevue, Washington, a developer of children's software, sponsors the Diabetes HomeCare Center. The site provides an electronic logbook as well as statistical and graphical analyses of the user’s blood glucose levels. The site has software that will download the data from several blood glucose meters and transmit it directly to the Web site.

Diabetes Research
The Diabetes Research online glucometer project is a free resource designed to assist in the self-management of diabetes. It's an interesting Java-based approach.

Dia-Log.com
Dia-Log.com, your On-Line Diabetes LogBook, lets you track your blood glucose, regular and long-lasting insulin, carbohydrates, and other foods. Entry is manual. It's now totally free.

DailyRating.com

DailyRating.com offers a question for you to answer on "What was your Blood Sugar level today?" All readings are in mmol/L, which we don't use in the U.S. and allow for only one reading a day. You can keep your ratings and journal private (the default) or make them public. The site produces line or bar charts from your readings. The neatest feature of this site is that you can update your daily reading by replying to a daily e-mail from the site. "You have the ability to create your own questions," CEO Calvin Tarlton <mailto:calvin@dailyrating.com> points out to me. "So, for example, if the current diabetes question is not tracking what you need, then you can easily create another one that tracks exactly what you want. In addition, you could set up multiple questions for different reading times."


Glucovance Daily Type 2 Diabetes Log
This online Daily Type 2 Diabetes Log lets you record all of your results on a daily basis, helping you see what does and does not work in your diet, exercise, and medication plan. The log calculates your daily and weekly averages and lets you know if you're within your target range. What this site does that I have never seen before is allowing you to return to update your numbers without logging in with a password, etc., as long and you use the same computer and browser.

Foot Wounds: Jeffrey Stone works with people who have chronic foot wounds, that is, wounds that have gone unhealed for more than two months. Many of the people Stone sees have diabetes. On average, the wounds of his patients with diabetes have been present for 11.8 months, and some have been there for years.
